Registration Form Please complete ONE registration form for each person attending the Workshop and/or Luncheon. Fill in the form completely. If you have a child attending please circle the registration fee for their age and if they will be attending the luncheon.
After completing the form you may fax it, e-mail with credit card payment, or mail it with a check to the fax number and address at the bottom of the form.
Please note: There is a Workshop registration fee; and an additional fee to attend the VYSA Awards Luncheon. A registration form must be filled out for each attendee.
