Roster Change Form (Release or Transfer):

  • VYSA Player Team Status Form - To be RELEASED you MUST complete sections 1, then give to your team manager.  If the form is not properly signed, it will delay the release.
    **VERY IMPORTANT -- A Release is NOT completed until section 2 (Release Section) is signed and stamped by the registrar**

    To be TRANSFERRED (going to a new team) you must complete section three on the release form you received from the player, or you may complete another Player Team Status Form with section 1 & 3 completed.  You will still need to submit a copy of the release form, along with the old player pass.

US Youth Soccer National Interstate Release, Guest Player (being released by one state to play in another)/Relocation Forms (Needs to be Approved by both State Associations):


  • Interstate Permission Form - To be used for Interstate Release, Guest Player, or Relocation Form  (A. For a player who lives in VA and wishes to play for a team in another State.  B. For a player who wishes to guest play with a Team outside of VA or a player from outside of VA to guest with a VA team. C. For a player that has moved from VA and was not released from their travel team or vice versa)

Maryland Players to Play in VA:

  • A Maryland player must first register on-line with Maryland State Youth Soccer Association each Seasonal Year.  You will need 5 copies of this printed form - no hand printed forms will be accepted.
  • The Maryland Player ID Number will now be just like the VA Player ID (First six letters of the players last name (if short fill in with # signs), last two digits of birth year, two digits of birth month, two digits of birth day, first three letters of the players first name, then the number 1
  • You will need the new player card with two pictures and two signatures. 
  • Proof of birth

 For West Virginia Players to Play in VA:
 
  • Register on the WVSA website:  http://www.wvsoccer.net/cgi-bin/artman/publish/cat_index_54.shtml, click on “The following link is for WV residents wanting to play on an out of state team.”  Through this link, they can pay their WV insurance fee online, or they may send a check with the player pass (payable to WVSA for $20).
     
  • Once registered, the player can be added to the team.  You will need to enter the player as an out of State player with their out of state player id.  Their player ID number is the exact same as Virginia’s, but you need to enter the number manually, so please review the format and enter it.
     
  • Print out the card and mail it and a copy of the proof of age to:  Jude Green, 103 Main Ave.,  Nitro, WV 25143.  If the family did not pay online, include a check for $20.  If you send the card and proof of age express mail, do NOT require a signature.  That will only delay the process.  The address listed has a slot in the door for the mail, providing a safe, secure place for it.  Please include a self-addressed stamped envelope for the items to be returned to you.  The best option is to have it mailed to the team manager
     
International Forms:

**ALL Players that were born outside the US MUST complete one of the form below**

FIFA requires all players over the age of 12, regardless of their ability or citizenship, to receive an international transfer clearance (“ITC”) from their former country when they are attempting to register to play soccer in a different country (i.e., the United States). U.S. Soccer is responsible for obtaining that ITC on behalf of all players in the U.S. participating in affiliated leagues.

FIFA, however, prohibits the transfer of players between the ages of 12 to 17 (minors) unless the player meets one of two exceptions:

(a) The player has moved with his/her parents to the U.S. for reasons other than playing soccer (e.g. work);
or (b) The player and prospective club are both located within 30 miles of an international border.

If the player can prove to FIFA that either of the two exceptions above has been met, U.S. Soccer would be permitted to request the ITC for that minor player. If the player does not meet one of the two exceptions above, the available options to register are listed below.

In 2009, FIFA instituted the FIFA TMS system. TMS is an online method to provide FIFA with documentation to prove that a player fits either of the two exceptions above. Documents are uploaded into FIFA’s system and are reviewed by FIFA, who then makes the final determination on whether an ITC request could be fulfilled for a minor player. However, U.S. Soccer has received a limited exemption from FIFA from the requirement to use the electronic FIFA TMS system. U.S. Soccer is now responsible for the documentation, review and determination of whether a player meets either of the two exceptions above. Although required to uphold the same standards as FIFA, U.S. Soccer is able to review the documents on an expedited basis and determine whether an ITC request could be made for a minor player.

International Transfer Clearance Options:
Under FIFA’s rules, there are four options by which a player born outside of the U.S. can register with an affiliated league:

1. Entry Prior to Age 12 (P12) - Players who entered the U.S. prior to the age of 12 (regardless of their current age) can supply a copy of an official document to prove that fact (e.g. – report cards, permanent resident card, medicare card, immunization records, doctor records).   http://www.ussoccer.com/About/Federation-Services/Intl-Clearance/Prior-12.aspx

a. U.S. Citizens – U.S. citizens born outside of the U.S. (regardless of current age) may simply complete and sign the First Registration form attesting that the player has never been registered at any level to play soccer in any other country. VYSA can immediately clear the player.

b. Non-U.S. Citizens – A player born outside of the U.S., who is currently over the age of 18, may also complete and sign the First Registration form. U.S. Soccer will contact the foreign federation for confirmation, and once that is received, U.S. Soccer can clear the player.

3. Minorshttp://www.ussoccer.com/About/Federation-Services/Intl-Clearance/Minors.aspx  - Any player currently between the ages of 12 to 17 who is NOT eligible for one of the proceeding methods must prove to U.S. Soccer that he/she meets one of the exceptions (a) or (b) above. Once verified, U.S. Soccer will request the ITC from the former federation. Per FIFA regulations, that federation has up to 30 days to respond after which U.S. Soccer can issue a provisional international transfer clearance.

In order to get registered under this process, the minor player (and parents) should follow these steps:  Collect required documentation to prove that the player meets the first of the above-given exceptions:

  1. Player’s Passport
  2. Player’s Birth Certificate
  3. Player’s Parents’ Passports
  4. Player’s Parents’ Work Visas (if available)
  5. Player’s Parents’ Proof of Residence (Water bill, lease agreement, etc.)
  6. Player’s Parents’ Proof of Employment (Pay Stub, Employment Verification Letter, etc.

ITC Request (ITC) – Players 18 years of age and over who are not eligible for the Entry Prior to Age 12 or First Registration options, will need to fill out the ITC Request Form and submit it to VYSA. VYSA will send the form on to U.S. Soccer will then request the ITC from the former federation. Per FIFA regulations, that federation has up to 30 days to respond after which U.S. Soccer can issue a provisional international transfer clearance.  http://www.ussoccer.com/About/Federation-Services/Intl-Clearance/itc-request.aspx

Permission to Travel:


International Paperwork for Travel:

Submit the following paperwork to the VYSA State Office for International Travel. Once VYSA approval is granted, the applications will be forward to US Soccer:
  • A completed Application for Foreign Travel (must be the original no copies or faxes)
  • A copy of the official brochure, pamphlet, invitation or applicable material for the Tournament. 
  • A copy of the approved roster which includes all players and team officials 
  • A $50 check per team payable to US Soccer 
  • A signed Ted Stevens Olympic and Amateur Sports Act signed 
  • VYSA On-line Permission to travel

Forms to Host a Tournament involving Foreign Teams:

Submit the following paperwork to the VYSA State Office 60 days prior to the tournament. Once approval is granted by VYSA, the applications will be forward to US Soccer:
