Required Forms for District Players
All District Players must complete a Code of Conduct form and Medical Release form before participating in the district ODP. Please download these forms and complete.
Please note:  These are the most current forms.  Do not use other versions from prior years.

    All forms are to be turned in to the team managers at the first training sessions.
    Training dates and times will be posted on the website.
